CONTRAST ENHANCED CT SCAN OF THORAX

Study Protocol: Contrast enhanced CT scan of Thorax was performed after intravenous administration of nonionic contrast medium using axial sections on Multi-slice CT scanner.

Clinical Profile Presented with right sided chest pain and breathlessness

Comparisons: No prior imaging is available for comparison.

FINDINGS:

Primary lesion: There is an irregular partially necrotic heterogeneously enhancing nodular mass is seen epicentered superior segment of right lower lobe involving the adjacent oblique fissure extending into the right upper and middle lobe measuring about 6.4 x 6.2 x 5.4 cm in maximum AP x TR x CC dimensions.

The lesions show spiculated margins with close abutment and suspicious involvement of the adjacent mediastinal and costal pleura. Few centrilobular airspace opacification and bronchiolar wall thickening is seen in left upper lobe, likely infective.

Local Extent:

Airways: There is encasement of the right primary bronchus with severe compression and cut off of the lobar bronchi resulting in airspace opacification and atelectatic changes in right upper and lower lobe.

Pleural surface: The adjacent mediastinal and costal pleura are abutted and suspiciously involved by the lesions with mild nodular pleural thickening and very minimal effusion. Multiple prominent to enlarged lymph nodes are also seen along the right costal pleura

Chest wall: Not Involved.

Vessels: The right inferior pulmonary artery is increased by the primary mass with rest of the vessels showing maintained.

Regional Extent:

Lymph nodes: Multiple enlarged pretracheal, paratracheal, right hilar and subcarinal nodes are seen largest measuring about 2.7 x 1.5 cm right subcarinal region. Enlarged metastatic appearing fluid is seen at left supraclavicular region measuring about 2.1 x 1.4 cm.
